Please allot approximately 10-30 minutes to complete all sections in this application entirely. Please note: The application is considered INCOMPLETE AND WILL NOT BE REVIEWED until the application fee of $50/child has been received. Please utilize one of the methods below for submission of the application fee.

We are committed at PARK PREP in the development and the education of your child. The following information will help us understand your child and his/her needs while continuing to support all family structures through the use of curriculum and an interactive community.

1. Please describe your child’s home and family and/or custodial arrangements below. Please include information if one/both of the parents are deceased or if the parents are divorced or separated. Who cares for your child when you are not at home? What other adult(s) influence your child’s life? Is there anything else you wish for the school to know about your child/family? 2.
